Who We Are
The Arc of Bristol County’s Adult Family Care (AFC) program offers residents of Massachusetts who need assistance with activities of daily living due to frailty, chronic illness, advanced age, or cognitive, physical or mental disability the opportunity to safely live in a family home. AFC gives participants an opportunity to live with trained, paid caregivers.
Eligibility:
In order to be eligible for AFC services, you must receive MassHealth (either Standard or CommonHealth), be at least 16 years old, and services must be ordered by a physician. You must also require supervision, cueing, or physical assistance with one or more activities of daily living (ADLs) and/or daily management of behaviors. ADLs include:
- Ambulation
- Eating/feeding
- Dressing/undressing
- Bathing
- Toileting
- Transferring
Requirements:
AFC participants reside with their caregivers, who may be parents, siblings, or unrelated adults aged 18 and older, capable of providing round-the-clock care in a qualified home environment. Caregivers cannot be the participant’s guardian or spouse and must undergo a thorough application process. This process includes CORI and SORI background checks, an RMV check, medical clearance, and a home assessment.
Services:
The Arc of Bristol County is dedicated to supporting AFC participants and caregivers, offering a unique, family-centered living experience. Each participant receives the assistance they need with personal care and activities of daily living, allowing them to remain as independent as possible. The Arc’s goal is to provide participants with full inclusion in their families and the broader community. AFC provides nursing and case management services, partnering with the participant and their caregiver to offer ongoing support and resources.
